    摘要:

    目的 探究天麻钩藤饮配合头部刮痧对原发性高血压伴睡眠障碍患者血压、匹兹堡睡眠质量指数量表(PSQI)评分及血清同型半胱氨酸(Hcy)、皮质醇(Cor)的影响。方法 选取2023年4月—2024年4月于南京中医药大学附属苏州市中医医院进行治疗的原发性高血压伴睡眠障碍患者120例为观察对象,采用区间随机法将其随机分为观察组和对照组,各60例。对照组患者给予常规西医治疗联合头部刮痧,观察组患者在对照组基础之上联合天麻钩藤饮。比较两组患者治疗前后中医症状积分、24 h收缩压变异性(SBPV)、白昼SBPV、夜间SBPV与24 h舒张压变异性(DBPV)、白昼DBPV、夜间DBPV、PSQI及多导睡眠图参数[实际睡眠时间、睡眠效率、睡眠维持率、睡眠潜伏期、快速眼动(REM)潜伏期、REM睡眠比例、夜间觉醒次数、觉醒总时间]、血清Hcy、Cor及血清单胺类神经递质[5-羟吲哚乙酸(5-HIAA)、5-羟色胺(5-HT)、β-内啡肽(β-EP)]水平。结果 治疗后,两组患者中医症状各项积分、PSQI总分均降低,且观察组患者评分更低(P<0.05);治疗后,两组患者实际睡眠时间、睡眠效率、睡眠维持率、睡眠潜伏期、REM潜伏期及5-HT、β-EP均上升,24 h SBPV、白昼SBPV、夜间SBPV、24 h DBPV、白昼DBPV、夜间DBPV、REM睡眠比例、夜间觉醒次数、觉醒总时间及血清Hcy、Cor、5-HIAA水平均降低,且观察组患者实际睡眠时间、睡眠效率、睡眠维持率、睡眠潜伏期、REM潜伏期及5-HT、β-EP更高,24 h SBPV、白昼SBPV、夜间SBPV、24 h DBPV、白昼DBPV、夜间DBPV、REM睡眠比例、夜间觉醒次数、觉醒总时间及血清Hcy、Cor、5-HIAA水平更低(P<0.05)。结论 采用天麻钩藤饮配合头部刮痧治疗原发性高血压伴睡眠障碍患者效果显著,能够有效控制血压水平,明显改善睡眠质量及多导睡眠图监测结果,其机制可能与调节Hcy、Cor水平及单胺类神经递质表达有关

    Abstract:

    Objective To explore the influence of Tianma Gouteng drink combined with head scraping on blood pressure, Pittsburgh Sleep Quality Index (PSQI) score, serum homocysteine (Hcy) and cortisol (Cor) in patients with essential hypertension and sleep disorders. Methods Totally 120 patients with essential hypertension and sleep disorders who received treatment in the hospital were selected from April 2023 to April 2024 as the research subjects, and were randomized into study group and control group by block randomization method, 60 cases in each group. The control group was treated with conventional western medicine combined with head scraping, while the study group was combined with Tianma Gouteng drink on the basis of the control group. The TCM symptoms scores, blood pressure variability [24h systolic blood pressure variability (SBPV), daytime SBPV, nighttime SBPV and 24h diastolic blood pressure variability (DBPV), daytime DBPV, nighttime DBPV], PSQI and polysomnography parameters [actual sleep time, sleep efficiency, sleep maintenance rate, sleep latency, rapid eye movement (REM ) latency, REM sleep ratio, nighttime awakening frequency, total awakening time), serum Hcy, Cor and serum monoamine neurotransmitters [5-hydroxyindoleacetic acid (5-HIAA), 5-hydroxytryptamine (5-HT), β-endorphin (β-EP)] were compared between groups before and after treatment. Results After treatment, the scores of TCM symptoms and total score of PSQI were decreased in both groups, and the scores in study group were lower (P<0.05). After treatment, the actual sleep time, sleep efficiency, sleep maintenance rate, sleep latency, REM latency, 5-HT5 and β-EP in both groups were enhanced while 24h SBPV, daytime SBPV, night SBPV, 24h DBPV, daytime DBPV, night DBPV, REM sleep ratio, nighttime awakening frequency, total awakening time and serum Hcy, Cor and 5-HIAA levels were declined, and the actual sleep time, sleep efficiency, sleep maintenance rate, sleep latency, REM latency, 5-HT5 and β-EP in the study group were higher while 24h SBPV, daytime SBPV, nighttime SBPV, 24h DBPV, daytime DBPV, nighttime DBPV, REM sleep ratio, nighttime awakening frequency, total awakening time and serum Hcy, Cor and 5-HIAA levels were lower (P<0.05). Conclusion Tianma Gouteng drink combined with head scraping has a significant therapeutic effect on patients with essential hypertension complicated with sleep disorders. It can effectively control blood pressure, and significantly improve sleep quality and polysomnography monitoring results. Its mechanism may be related to the regulations of Hcy, Cor and monoamine neurotransmitters
