Effect of Bikangfu nasal tamponade therapy combined with Tongqiao rhinitis granules on inflammatory indexes and transforming growth factor-β1 in pediatric patients with chronic rhinosinusitis

    摘要:

    目的 探讨鼻康复鼻腔填塞疗法联合通窍鼻炎颗粒治疗对儿童慢性鼻窦炎(CRS)患者炎性指标及转化生长因子-β1(TGF-β1)的影响。方法 选取2023年4月—12月河南省中医院耳鼻喉科收治的108例确诊CRS的患儿为研究对象,采用随机数字表法分为对照组与观察组,每组54例。对照组在常规治疗基础上给予通窍鼻炎颗粒,每袋2 g,每次1/2袋,每日3次,开水冲服。观察组在对照组的基础上给予鼻康复鼻腔填塞疗法治疗,每日1次,每次留置20 min。治疗结束后,比较两组患儿血清及鼻分泌液中炎性因子[白三烯C4(LTC4)、白细胞介素17(IL-17)]水平,炎症相关蛋白[嗜酸性粒细胞阳离子蛋白(ECP)、水通道蛋白-3(AQP-3)]水平,血清中IgG、IgE和T淋巴亚群变化及TGF-β1通路关键基因Smad3、Smad7、α-SMA、TGF-β1及TGF-β3表达量变化。结果 治疗后,观察组患儿血清、鼻分泌液中LTC4、IL-17及ECP降低,血清中IgE、CD8+水平降低,Lund-Kennedy鼻内镜检查量表评分降低,且均低于对照组(P<0.05);观察组患儿血清、鼻分泌液中AQP-3增高,血清中IgG、CD3+、CD4+及CD4+/CD8+水平增高,且均高于对照组(P<0.05)。治疗后,观察组TGF-β1通路Smad3基因表达上调,Smad7、α-SMA、TGF-β1、TGF-β3基因表达下调(P<0.05)。两组患儿不良反应发生情况无统计学差异(P>0.05)。结论 鼻康复鼻腔填塞疗法联合通窍鼻炎颗粒治疗可降低LTC4、IL-17、ECP、IgE及CD8+水平,增高AQP-3、IgG、CD3+、CD4+及CD4+/CD8+水平,改变T淋巴亚群结构。联合治疗可缓解患儿体内炎症反应,降低患儿体内TGF-β1的表达水平

    Abstract:

    Objective To investigate the effect of Bikangfu nasal tamponade therapy combined with the treatment of Tongqiao rhinitis granules on the inflammatory indicators and transforming growth factor-beta 1 of chronic rhinosinusitis in children. Methods One hundred and eight children with confirmed diagnosis of chronic sinusitis were selected for the study and were divided into 54 cases in the control group and 54 cases in the observation group using randomization method. The control group was given Tongqiao rhinitis granules, 2 g per sachet, 1/2 sachet each time, 3 times a day with boiled water. The observation group was treated with Bikangfu nasal tamponade therapy based on the control group, which was left in place for 20 min once a day. At the end of treatment, the levels of inflammatory factors (LTC4, IL-17) and inflammation-related proteins (ECP, AQP-3) in serum and nasal secretion fluid, changes in IgG, IE and T-lymphoid subgroups in serum and changes in the expression of key genes of the TGF-β1 pathway, Smad3, Smad7, αSMA, TGF β1, and TGFβ3 were compared between the 2 groups of children. 〖WTHZ〗Results The levels of LTC4, IL-17, ECP, IgE, and CD8+ in the serum and nasal secretions of the observation group were decreased, and the Lund-Kennedy endoscopy score was also reduced, all of which were lower than those in the control group (P<0.05). The levels of AQP-3, IgG, CD3+, CD4+, CD4+/CD8+ in the serum and nasal secretion fluid of the children in the observation group were increased and were higher than those in the control group (P<0.05). TGF-β1 pathway Smad3 gene expression was up-regulated and Smad7, α-SMA, TGFβ1, TGF-β3 gene expression was down-regulated after treatment(P<0.05). There was no statistical significance in the occurrence of adverse reactions in children in the 2 groups (P>0.05).Conclusion Bikangfu nasal tamponade therapy combined with Tongqiao rhinitis granules combination therapy can decrease LTC4, IL-17, ECP, IgE, and CD8+ levels, increase AQP-3, IgG, CD3+, CD4+, CD4+, and CD4+/CD8+ levels, and altere the structure of T-lymphoid subpopulations. The combination treatment alleviates the inflammatory response and reduces the expression level of TGF-β1 in the children
