    摘要:

    目的 探究动态对比增强磁共振成像(DCE-MRI)在子宫肌瘤诊断及病理分级评估中的应用价值。方法 共纳入我院2021年1月—2023年1月收治的605例疑似子宫肌瘤患者,以术后病理结果为金标准,评估DCE-MRI诊断子宫肌瘤及其病理分型和良恶性的价值。结果 以术后病理结果为金标准,DCE-MRI诊断子宫肌瘤的灵敏度为88.41%、特异度为95.90%、准确度为89.92%,阳性预测值为98.84%、阴性预测值为67.63%,Kappa值为0.869。富细胞型子宫肌瘤患者Kep、Ve、Ktrans高于普通型、退变型(P<0.05)。DCE-MRI定量参数Kep、Ve、Ktrans联合检测预测富细胞型子宫肌瘤的曲线下面积(0.878)、特异度(96.47%)较高。子宫肉瘤患者DCE-MRI定量参数Kep、Ve、Ktrans高于良性变性子宫肌瘤(P<0.05)。DCE-MRI定量参数Kep、Ve、Ktrans联合检测预测子宫肉瘤的曲线下面积(0.867)、敏感性(82.54%)较高。结论 DCE-MRI在子宫肌瘤诊断及病理分级和良恶性评估中有较高应用价值

    Abstract:

    Objective To explore the application value of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) in the diagnosis and pathological grading evaluation of uterine fibroids. Methods 605 patients who were suspected of uterine fibroid and admitted to the hospital from January 2021 to January 2023 were included in this study. With postoperative pathological results as the gold standard, the value of DCE-MRI in the diagnosis and pathological classification of uterine fibroids was evaluated. Results With postoperative pathological results as the gold standard, the sensitivity, specificity, accuracy, positive predictive value, negative predictive value and Kappa value of DCE-MRI for diagnosing uterine fibroids were 88.41%, 95.90%, 89.92%, 98.84%, 67.63% and 0.869, respectively. Kep, Ve and Ktrans of patients with cellular uterine fibroids were significantly higher than those of patients with normal and degenerative uterine fibroids (P<0.05). The area under the curve and specificity of the combination of Kep, Ve and Ktrans for predicting cellular uterine fibroids (0.878 and 96.47%) were higher. Kep, Ve and Ktrans of patients with uterine sarcoma were significantly higher than those of patients with benign degenerative uterine fibroids (P<0.05). The area under the curve and sensitivity of the combination of Kep、Ve and Ktrans for predicting uterine sarcoma (0.867 and 82.54%) were higher. Conclusion DCE-MRI is of high value in the diagnosis and pathological grading evaluation of uterine fibroids
