乳腺癌X线征象与ER、PR、HER-2、Ki-67表达及分子亚型的相关性

    摘要:

    目的 探讨乳腺癌X线征象与雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HER-2)、增殖细胞核抗原(Ki-67)及分子亚型的相关性。方法 回顾性分析2020年9月~2021年1月我院收治的经病理证实的226例乳腺癌患者的影像及病理资料,采用χ2检验或Fisher确切概率法检验,分析乳腺癌X线征象与ER、PR、HER-2、Ki-67表达及分子亚型的关系。结果 肿块征、分叶征HER-2多为阴性(P=0.01、P=0.04);钙化征HER-2多为阴性、Ki-67多>14%(均P<0.01);毛刺征ER和PR多为阳性、Ki-67多>14%(P<0.01、P=0.02、P<0.01);凹陷征、病灶≥2cm的Ki-67多>14%(P=0.03、P=0.02)。肿块伴或不伴钙化、结构扭曲HER-2阴性多见,单纯钙化HER-2过表达相对多见,肿块伴或不伴钙化、单纯钙化、结构扭曲均是Ki-67>14%多见(均P<0.01)。钙化征Luminal B1型、HER-2过表达型多见,毛刺征Luminal B1型、Luminal A型多见(均P<0.01)。单纯肿块Luminal A型、Luminal B1型多见,肿块伴钙化Luminal B1型多见,单纯钙化HER-2过表达型、Luminal B1型多见,而TNBC型最少,结构扭曲Luminal A型、TNBC型相对多见(P<0.01)。结论 乳腺癌X线征象可以为乳腺癌的免疫组化指标和分子亚型提供相关信息。

    Abstract:

    Objective To investigate the correlation between X-ray Mammography characteristics and ER, PR, HER-2, Ki-67 expression and molecular subtypes of breast cancer. Methods The imaging and pathological data of 226 patients with breast cancer confirmed by pathology were analyzed retrospectively. χ2 test or Fisher's exact probability test was used to analyze the difference of the X-ray Mammography characteristics of breast cancer in ER, PR, HER-2, Ki-67 expression and molecular subtypes. Results The mass sign and the lobular sign were mostly negative for HER-2(P=0.01, P=0.04). The calcification sign was mostly negative for HER-2 and more than 14% for Ki-67(all P<0.01). The glitch sign was mostly positive for ER, PR and more than 14% for Ki-67(P<0.01, P=0.02, P<0.01); The depression sign and the lesion≥2 cm were more than 14% for Ki-67(P=0.03, P=0.02). Mass with or without calcification, structural distortion are more common in HER-2negative, simple calcification is more common in HER-2 overexpression, mass with or without calcification, simple calcification and structural distortion are all more common in Ki-67>14%(all P<0.01). Luminal B1and HER-2overexpression are more common in calcification sign, Luminal B1and Luminal Aare more common in glitch sign(P<0.01). Luminal A and Luminal B1are more common in simple mass., Luminal B1is more common in mass with calcification, HER-2 overexpression and Luminal B1are more common in simple calcification, while TNBC is the least,Luminal A and TNBC are relatively common in structural distortion(P<0.01). Conclusion The X-ray Mammography characteristics can provide relevant information for the immunohistochemical indexes and molecular subtypes of breast cancer.
