    目的 探究宫颈癌患者经阴道彩色多普勒超声(TVCDS)图像特征和血流动力学参数变化,分析TVCDS检查与血流动力学参数变化在宫颈癌临床诊断中的应用价值。方法 选取2018年1月~2021年1月本院收治的110例宫颈癌患者为宫颈癌组,另选取同期在本院接受体检的健康志愿者97例为对照组,所有受试者均接受TVCDS检查,比较宫颈癌组与对照组的彩超图像特征和血流动力学参数,比较宫颈癌组不同血流信号分级患者的彩超图像特征和血流动力学参数变化,分析血流信号分级与血流动力学参数的相关性。结果 宫颈癌患者的阴道彩超图像特征主要为宫颈前后径增大、轮廓改变,回声以低回声为主,同时可见等回声或高回声,CDFI可见病灶区有丰富血流信号;宫颈癌组患者的宫颈偏大、低回声、高回声比例明显高于对照组,RI水平明显小于对照组,PSV和EDV水平明显大于对照组(P<0.05)。不同临床分期、血流信号分级患者的血流动力学参数比较,差异均有统计学意义(P<0.05)。相关性分析结果显示,RI与血流信号分级呈明显负相关,PSV、EDV与血流信号分级呈明显正相关(P<0.05)。结论 TVCDS在检测宫颈癌血流动力学参数方面具有较高的价值,血流动力学参数与临床分期、血流信号分级相关,对宫颈癌患者的阴道彩超图像特征和血流动力学参数变化进行评估,可为临床宫颈癌的诊治提供一定指导意义。

    Abstract:

    Objective To explore the images characteristics of transvaginal color Doppler sonography (TVCDS) and changes of hemodynamic parameters in patients with cervical cancer,and analyze the application value of TVCDS and changes of hemodynamic parameters in the clinical diagnosis of cervical cancer. Methods A total of 110 patients with cervical cancer admitted to the hospital from January 2018 to January 2021 were enrolled as cervical cancer group,while other 97 healthy volunteers undergoing physical examination during the same period were enrolled as control group. All underwent TVCDS. The image characteristics of TVCDS and hemodynamic parameters between cervical cancer group and control group,the image characteristics of TVCDS and changes of hemodynamic parameters among patients with different blood flow signal grading in cervical cancer group were compared. The correlation between blood flow signal grading and hemodynamic parameters was analyzed. Results The image characteristics of TVCDS in patients with cervical cancer were mainly characterized by increased anterior and posterior diameters of the cervix,and changes in contour. The echo was mainly on lowecho,there was equal echo or highecho. CDFI showed that there were abundant blood flow signals in lesion area. The proportions of rather large cervix,lowecho and highecho in cervical cancer group were significantly higher than those in control group,RI level was significantly lower than that in control group,PSV and EDV were significantly higher than those in control group (P<0.05). There were statistically significant differences in hemodynamic parameters among patients at different clinical stages and with different blood flow signal grading (P<0.05). Correlation analysis showed that RI was significantly negatively correlated with blood flow signal grading,while PSV and EDV were significantly positively correlated with it (P<0.05). Conclusion TVCDS is of high value in detecting hemodynamic parameters of cervical cancer. Hemodynamic parameters are related to clinical stage and blood flow signal grading. The evaluation on images characteristics of TVCDS and changes of hemodynamic parameters in patients with cervical cancer can provide certain guidance significance for clinical diagnosis and treatment of cervical cancer.
