    目的 探讨增强超声(CEUS)对BI-RADS 4类乳腺肿块的优化再评估作用。方法 筛选2018年9月~2020年3月在我院经手术病理证实的174例患者180个常规超声评估为BI-RADS 4类以上的乳腺肿块,基于CEUS的增强模式特征分析并对乳腺肿块分类进行再评估调整,并与病理结果进行比较,采用受试者工作(ROC)曲线评价CEUS对BI-RADS 4类以上乳腺肿块的风险再评估价值。结果 180个乳腺肿块中,恶性肿块99个,良性肿块81个。调整前BI-RADS分类对恶性肿块诊断的敏感度、特异度、准确度、阳性预测值和阴性预测值分别为90.91%、80.25%、86.11%、84.91%、87.84%;基于CEUS调整后的BI-RADS分类对恶性肿块诊断的敏感度、特异度、准确度、阳性预测值和阴性预测值分别为 94.95%、81.48%、88.89%、86.24%、92.96%。调整后的 BI-RADS分类的ROC曲线下面积优于调整前(0.947 vs 0.900,P<0.05)。结论 结合CEUS对常规超声BI-RADS分类具有一定的优化再评估作用,能进一步明确性质,避免漏诊,亦减少不必要的活检,可在一定程度上提高超声医师对乳腺肿块的诊断信心。

    Abstract:

    Objective To investigate the effect of CEUS on the optimal reevaluation of BI-RADS 4 breast masses.Methods 174 patients with 180 breast masses were screened as BI-RADS type 4 or above by conventional ultrasound evaluation. Their classification was re-evaluated and adjusted based on the feature analysis of CEUS, and compared with pathological results. The subject operating characteristic (ROC) curve was used to evaluate the risk reassessment value of CEUS for breast masses above BI-RADS 4.Results Among 180 breast masses, 99 were malignant and 81 were benign. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of BI-RADS classification for malignant tumor diagnosis before adjustment were 90.91%, 80.25%, 86.11%, 84.91% and 87.84%, respectively. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of BI-RADS classification adjusted by CEUS in the diagnosis of malignant masses were 94.95%, 81.48%, 88.89%, 86.24% and 92.96%, respectively. The area under the ROC curve of BI-RADS classification after adjustment was 0.947, which was better than 0.900 before adjustment (P<0.05).Conclusion Combined with CEUS, it plays a certain role in optimizing and reevaluating the classification of conventional ultrasound BI-RADS, further clarifying the nature, avoiding missed diagnosis and reducing unnecessary biopsies, which will improve the diagnostic confidence of ultrasound doctors in breast masses to a certain extent.
