经鼻高流量联合新活素对肺源性心脏病患者NT-proBNP肌钙蛋白及CKMB的作用

    【摘要】 目的 探讨经鼻高流量吸氧联合新活素对肺源性心脏病患者N末端B型利钠肽原(NT-proBNP)、肌钙蛋白、肌酸激酶同工酶MB(CKMB)的作用。 方法 选择2019年1月~2020年1月在廊坊市人民医院接受治疗的100例肺源性心脏病患者,采用抽签法分为观察组(n=53)和对照组(n=47)。对照组给予新活素治疗,观察组在对照组的基础上给予经鼻高流量吸氧治疗。比较两组患者的临床疗效、NT-proBNP、肌钙蛋白、CKMB、左室射血分数、每分输出量、6分钟步行距离、肺总量(TLC)、用力肺活量(FVC)、第一秒用力呼气量占用力肺活量比值(FEV1/FVC)变化情况。 结果 两组患者总有效率差异有统计学意义(P<0.05)。治疗前,两组NT-proBNP、肌钙蛋白、CKMB水平差异均无统计学意义(P>0.05);治疗后,两组血清NT-proBNP、肌钙蛋白、CKMB水平均显著下降,且观察组低于对照组(均P<0.05)。治疗前,两组左室射血分数、每分输出量、6分钟步行距离水平差异均无统计学意义(均P>0.05);治疗后,两组左室射血分数、每分输出量、6分钟步行距离水平均显著升高,且观察组高于对照组(P<0.05);治疗前,两组TLC、FVC、FEV1/FVC水平差异均无统计学意义(均P>0.05);治疗后,两组TLC、FVC、FEV1/FVC水平均显著升高,且观察组高于对照组(均P<0.05)。 结论 在肺源性心脏病患者中应用经鼻高流量吸氧联合新活素治疗效果显著,可有效改善患者NT-proBNP、肌钙蛋白、CKMB水平。

    【Abstract】 Objective To study the effect of nasal high flow combined with neoactivin on NT-proBNP troponin and CKMB in patients with pulmonary heart disease. Methods 100 patients with pulmonary heart disease treated in our hospital from January 2019 to January 2020 were divided into observation group (n=53) and control group (n=47). The control group was given Xinhuosu treatment, and the observation group was given nasal high flow oxygen inhalation on the basis of the control group. The clinical efficacy, NT-proBNP, troponin, CKMB, left ventricular ejection fraction, output per minute, 6-minute walking distance, total lung capacity (TLC), forced vital capacity (FVC), FEV1/FVC were compared between the two groups. Results The difference of total effective rate between the two groups was statistically significant (P<0.05). Before treatment, there was no significant difference in the levels of NT-proBNP, troponin and CKMB between the two groups (P>0.05). After treatment, the levels of serum NT-proBNP, troponin and CKMB in the two groups were significantly decreased, and the levels in the observation group were lower than those in the control group (all P<0.05). Before treatment, there was no significant difference in left ventricular ejection fraction, output per minute and 6minute walking distance between the two groups (P>0.05). After treatment, the levels of left ventricular ejection fraction, output per minute and 6minute walking distance in the two groups were significantly increased, and the levels of TLC, FVC, FEV1/FVC in the observation group were significantly higher than those in the control group (P<0.05) After treatment, the levels of TLC, FVC, FEV1/FVC in the two groups were significantly higher than those in the control group (P<0.05). Conclusion In patients with pulmonary heart disease, the application of transnasal high flow combined with neocortin has a significant effect, which can effectively improve the level of ntprobnp, troponin and CKMB.
