    【摘要】 目的 探讨丹参川芎嗪注射液对局部晚期非小细胞肺癌患者放射性肺损伤的防治效果。方法 选择2016年12月~2017年11月于我院进行治疗的局部晚期非小细胞肺癌患者80例,按照随机数字表法分为对照组40例和观察组40例。两组均进行三维适形放射治疗,观察组在此基础上给予丹参川芎嗪注射液。比较两组放射疗效,治疗前后血浆转化生长因子β1(TGFβ1)水平、血清细胞间黏附分子(ICAM1)水平、肺功能指标[用力肺活量(FVC)、第1秒用力呼气量(FEV1)及一氧化碳弥散量(DLCO)];对两组进行影像学检查,记录放射性肺损伤情况,并统计其不良反应发生情况。结果 两组有效缓解率比较差异无统计学意义(P>0.05)。治疗后,观察组血浆TGFβ1和血清ICAM1水平均低于对照组(P<0.05);观察组FVC、FEV1水平与对照组比较差异无统计学意义(P>0.05);观察组DLCO水平高于对照组(P<0.05)。观察组放射性肺炎及放射性肺纤维化发生率均低于对照组(P<0.05)。两组不良反应比较差异无统计学意义(P>0.05)。结论〓丹参川芎嗪注射液可改善局部晚期非小细胞肺癌患者肺弥散功能,减少放射性肺损伤。

    Abstract:

    【Abstract】 Objective To investigate the control effect of Danshen Chuanxiongqin injection on radiationinduced lung injury in patients with locally advanced nonsmall cell lung cancer (NSCLC). Methods Eighty patients with locally advanced NSCLC treated in the hospital from December 2016 to November 2017 were divided into the control group and the observation group by the random number table method, 40 cases in each group. Both groups were given radiotherapy, and on the basis, the observation group was additionally treated with Danshen Chuanxiongqin injection. Effects of radiotherapy, levels of plasma transforming growth factorβ1 (TGFβ1), serum intercellular adhesion molecule (ICAM1) and lung function indicators [forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1) and carbon monoxide diffusion capacity (DLCO)] were compared between the two groups. Imaging examinations were performed on both groups, and the incidence of adverse reactions was recorded. Results There was no significant difference between the effective remission rate between the two groups (5750% vs 5000%) (P>0.05). Levels of plasma TGFβ1 and serum ICAM1 in the observation group after treatment were lower than those in the control group (P<0.05), but there was no significant difference in FVC or FEV1 (P>0.05). The DLCO level was higher than that in the control group (P<0.05). The incidence rates of radiation pneumonitis and radiationinduced pulmonary fibrosis in the observation group (1250% and 1000%) were lower than those in the control group (3250% and 3250%) (P<0.05). There was no significant differences in adverse reactions between the two groups (P>0.05). Conclusion Danshen Chuanxiongqin injection can improve the lung diffusion function and reduce radiationinduced lung injury in patients with locally advanced NSCLC.
