Abstract:【Abstract】 Objective To analyze the effect of pancreaticoduodenectomy which combined with resection and reconstruction portal vein and common hepatic artery.Methods A case of cholangiocarcinoma involving portal vein and common hepatic arterial was reported. The improvement of the surgical resection rate and postoperative efficacy were explored.Results The operation time of the patient was 380min. Bleeding volume 400ml. there was no postoperative complications such as pancreatic leakage, biliary leakage, bleeding and abdominal infection occurred. The patient was discharged from hospital. She was hospitalized for 13 days. Abdominal enhanced CT was reviewed 4 days and 12 days after surgery. The anastomosis of portal vein and total hepatic artery was unobstructed. Followup examination has conducted for 3 months, and no distant metastasis was found . Conclusion In pancreaticoduodenectomy, portal vein and common hepatic artery invasion is not a contraindication for surgical,but the surgeon must be skilled in vascular surgery. The patient treated with pancreaticoduodenectomy, associating with portal vein and common hepatic arterial resection and reconstruction.
