Abstract:【Abstract】 Objective The aim of this study was to evaluate the efficacy, safety of tacrolimus and cyclosporine in the treatment of Membranous Nephropathy. Methods Cochrane Library, EMbase, OVID,PUBMED, CNKI, VIP and Wangfang Date were retrieved to collect the randomized controlled trials of tacrolimus (TAC) versus cyclosporine (CTX) in the treatment of Membranous Nephropathy(MN). Two reviewers independently screened literature, extracted data, and assessed the methodological quality of included studies. Metaanalysis was performed by using Rev Man 52. Results A total of 22 RCT involving 1286 patients were included. The Metaanalysis results showed that there were significant differences among the 6 month complete remission rate[RR=171,95%CI(148200),P<000001]and total 6 month remission rate[RR=133, 95%CI(124141),P<000001]in 2 groups. With the prolonged of course,there were no significant differences among the remission rate in 2 groups[RR=107,95%CI(096120),P=019]. Adverse reactions, TAC terms of Nephrotoxicity, hyperglycemia, Hand tremble were significantly higher than the CTX group(P≤005);Liver toxicity,Hair loss,Gastrointestinal reactions,the abnormal bone marrow suppression were significantly lower than the CTX group(P≤005). The risk of 6 month infections, Herpes zoster, hypertension gout were no significant differences in the two groups. Conclusion In 6 months,TAC shows higher efficacy than CTX in the treatment of MN,while with the extension of treatment, this advantage is reduced. Due to the less quantity and low quality,it remains to be further verified by RCT with large sample and high quality.
