门冬胰岛素30强化治疗2型糖尿病对血糖、血脂、胰岛β细胞功能的影响

    【摘要】 目的 探讨门冬胰岛素30强化治疗2型糖尿病的临床疗效及对血糖、血脂、胰岛β细胞功能的影响。 方法 选择2014年1月~2016年7月我院接诊的92例2型糖尿病患者,按照治疗方式不同分为观察组(n=46)和对照组(n=46)。对照组使用门冬胰岛素+精蛋白锌人胰岛素强化治疗,观察组在三餐前给予门冬胰岛素30强化治疗。比较两组血糖、血脂、胰岛素抵抗指数(HOMAIR)、胰岛β细胞分泌指数(△I30/△G30)、血糖达标时间、胰岛素用量以及低血糖的发生率。结果 治疗后,两组糖化血红蛋白(HbA1C)、空腹血糖(FBG)、餐后血糖(PPG)、甘油三酯(TG)、总胆固醇(TG)、低密度脂蛋白胆固醇(LDLL)、HOIMIR、△I30/△G30较治疗前均明显改善(P<005);两组治疗后HbA1C、FBG、PPG、TG、TC、LDLC、HOIMIR、△I30/△G30比较均无明显差异(P>005);观察组血糖达标时间短于对照组(P<005);两组胰岛素用量、低血糖发生率比较差异无统计学意义(P>005)。结论 在2型糖尿病患者应用传统胰岛素强化方案和门冬胰岛素30强化方案均可有效降低血糖、血脂水平,改善胰岛β细胞功能,但门冬胰岛素30强化治疗可缩短血糖达标时间,临床应用价值较高。

    【Abstract】 Objective To study curative efficacy of insulin aspart 30 in treatment of type 2 diabetes mellitus intensive treatment and its effects on blood sugar and blood lipid and islet βcell secretary function. Methods 92 patients of type 2 diabetes mellitus received therapy from January 2014 to July 2016 in our hospital were selected. According to random number table, those patients were divided into the observation group (n=46) and the control group (n=46). The control group was treated with insulin aspart + protamine zinc insulin intensive treatment, while the observation group was treated with insulin aspart 30 intensive treatment. The blood sugar, blood lipid, insulin resistance index (HOMAIR), islet βcell secretary function(△I30/△G30), time of reaching target of glucose, insulin dosage and incidence of hypoglycemia were compared. Results After treatment, hemoglobin (HbA1C), fasting plasma glucose (FBG), postprandial plasma glucose (PPG), triglyceride (TG), total cholesterol (TG), low density lipoprotein cholesterol (LDLL), HOIMIR, △I30/△G30 after treatment of the two group were significantly improved than that before treatment (P<005). There was no significant difference in the HbA1C, FBG, PPG, TG, TC, LDLC, HOIMIR and △I30/△G30 after treatment between two groups(P>005). The time of reaching target of glucose in the observation group was significantly shorter than that of the control group (524±116d, 782±159d). The difference was statistically significant (P<005). There was no significant difference between the two groups in the use of insulin dosage and the incidence of hypoglycemia(P>005). Conclusion Traditional insulin strengthening scheme and insulin aspart 30 all well for type 2 diabetes mellitus intensive treatment can effectively reduce the levels of blood sugar and blood lipid, improve the islet βcell secretary function. The insulin aspart 30 can shorten the time of blood glucose to reach the standard.
