    【摘要】目的 了解肺炎克雷伯菌耐药性趋势及临床分布特点, 为临床合理应用抗菌药物提供依据。方法 分析2013~2016年临床标本分离的3276株肺炎克雷伯菌资料, 应用WHONET 5.5软件和SPSS 13.0软件进行数据统计学分析。结果 3276株肺炎克雷伯菌主要来源于呼吸道标本(38.7%)与尿液(21.9%), 病区分布肝胆外科居首位(9.8%)。肺炎克雷伯菌对氨苄西林高度耐药, 耐药率达99%以上;对碳青霉烯类、头霉素类、哌拉西林他唑巴坦、头孢吡肟、头孢哌酮舒巴坦、妥布霉素、阿米卡星等抗菌药物的耐药率明显上升(P<0.01);对硝基呋喃类和磺胺类抗菌药物的耐药率明显下降(P<0.01);对替加环素高度敏感, 耐药率小于1%。多重耐药及耐碳青霉烯类肺炎克雷伯菌检出率明显上升(P<0.01), 泛耐药肺炎克雷伯菌检出率先下降后上升(P<0.01)。结论 多重耐药、泛耐药及耐碳青霉烯类肺炎克雷伯菌检出率呈上升趋势, 临床应引起重视, 及时采取感控措施, 实施抗菌药物合理应用管理方案。

    【Abstract】Objective To investigate the trend of antibacterial resistance and clinical distribution characteristics of Klebsiella pneumoniae from 2013 to 2016 and provide guidance for clinical rational use of antibacterial agents. Methods The reference of Klebsiella pneumoniae isolated from clinical specimens was analyzed retrospectively by WHONET 5.5 and SPSS 13.0 software from 2013 to 2016. Results Respiratory tract secretion and urine were the most common specimen source, accounting for 38.7% and 21.9% respectively. 9.8% of Klebsiella pneumoniae were isolated from department of hepatobiliary surgery, ranking first among all the departments. Klebsiella pneumoniae strains were highly resistant to ampicillin, and the resistance rate reached above 99%. The resistance rates of these strans to Carbapenems, cephamicins, piperacillintazobactam, cefepime, cefoperazonesulbactam, tobramycin and amikacin increased significantly (P<0.01). The resistance rates of these strans to Nitrofurans and sulfonamides decreased significantly (P<0.01). Above 99% of these strains were susceptible to tigecycline. The detection rates of muti-drug resistant and Carbapenemresistant Klebsiella pneumonia increased significantly (P<0.01). The detection rate of extensivelydrug resistant Klebsiella pneumonia firstly decreased then increased significantly (P<0.01). Conclusion The detection rates of mutidrug resistant, extensivelydrug resistant and carbapenemresistant Klebsiella pneumonia appeared increasing trend. Clinical workers should pay more attention and strengthen infection control. Department of surgery and ICU were the focus of infection prevention and control.
