    【摘要】目的 评价关节镜微骨折术治疗退行性膝关节软骨损伤患者的2年效果。方法 回顾性分析2012年6月~2014年8月我院35例退行性膝关节软骨损伤患者行关节镜微骨折术治疗的2年随访结果,观察其治疗有效率、住院时间、显性失血量、膝关节疼痛视觉模拟评分(VAS)、Hss膝关节功能评分、Tegner运动功能评分、膝关节曲度等预后情况,对比患者手术前后浮髌试验阳性率、髌股关节研磨试验阳性率,同时将患者分为改善组(显效+有效)、未改善组(无效),分析治疗效果的影响因素。结果患者治疗有效率为77.14%,住院时间、显性失血量分别为(7±1)d、(90±11)ml,术后患者膝关节VAS评分、浮髌试验阳性率、髌股关节研磨试验阳性率均较术前明显下降(P<0.05);术后患者Hss膝关节功能评分、Tegner运动功能评分、膝关节曲度均低于术前(P<0.05);改善组年龄、病程、软骨损伤程度、半月板损伤与未改善组比较,差异均具有统计学意义(P<0.05),Logistic回归分析显示年龄、病程、半月板损伤为影响关节镜微骨折手术的独立危险因素。结论 退行性膝关节软骨损伤行关节镜微骨折术疗效较好,可改善患者膝关节功能及运动功能,但也可能因年龄、病程、半月板损伤等因素而使疗效不佳,临床应采取相应防治措施。

    【Abstract】Objective To evaluate on degenerative knee cartilage injury in 2 years. Methods The 2-year follow-up results of 35 patients with degenerative knee cartilage injury treated by arthroscopic microfrature surgery in our hospital from June 2014 to August 2014 were retrospectively analyzed. The therapeutic efficacy, length of hospital stay, revealed blood loss, knee pain visual analogue scale (VAS) scores, Hss knee function scores, Tegner motor function scores and curvature of knee were observed. The positive rate of floating patella test and positive rate of patellofemoral joint grinding test were compared before and after surgery. The patients were divided into the improvement group (markedly effective and effective) and the nonimprovement group (ineffective). The influencing factors of the treatment effects were analyzed. Results The treatment efficiency, length of hospital stay and revealed blood loss were 77.14%, 7±1 d and 90±11 ml, respectively. After surgery, the knee joint VAS score, positive rate of floating patella test and positive rate of patellofemoral joint grinding test were significantly lower than those before surgery (P<0.05). After surgery, Hss knee joint function score, Tegner motor function score and knee curvature were also significantly lower than those before surgery (P<0.05). Age, duration of disease, degree of cartilage injury and meniscus injury showed statistically significant differences between the improvement group and the nonimprovement group (P<0.05). Logistic regression analysis showed that age, duration of disease and meniscus injury were independent risk factors of the efficacy of arthroscopic micro-fracture surgery (P<0.05). Conclusion The arthroscopic fracture microsurgery is effective in the treatment of degenerative knee cartilage injury. It can improve the knee function and motor function in patients, and the effect is influenced by age, course of disease and meniscus injury. Therefore, preventive measures should be taken.
