Abstract:【Abstract】 Objective To summarize the general features, the clinical manifestations and signs, checking methods, treatment and clinical outcome of 28 patients with infective endocarditis and improve the diagnosis and treatment level of infective endocarditis.Methods 28 cases of IE patients admitted to The First Affiliated Hospital of Chongqing Medical University from January 2010 to December 2015 were retrospectively analyzed, including the patient′s general characteristics, heart disease, clinical manifestation, routine laboratory ests, pathogenic examination, echocardiography table, course of treatment and clinic outcome.Results The average age of the patients was (4161±1460) years. The ratio of male to female was 155∶1. 4286%(12/28) of the cases at least accompanied by a basic heart disease, and the congenital ventricular septal defect (VSD) was the most common (6667%,8/12). The most common clinical manifestations were fever (6786%, 19/28) and the heart tired shortness of breath(6786%, 19/28). The positive rate of echocardiogram was 6786%(19/28). 5000% (14/28) of the patients detected positive pathogens, and their pathogens were more common in Streptococcus species (7143%,10/14). The most common antibiotic was penicillin (6429%,18/28). 2500% (7/28) patients were accepted surgical treatment. 8571% (24/28) patients in the cases had at least one complication. The inhospital mortality was about 714% (2/28).Conclusion Congenital heart disease is the most common predisposing factor of IE and the specific clinical manifestations and signs are rare. As the specific indicators have not yet been found,the diagnose still bases on culture of blood and echocardiogram. The pathogen of IE has no obvious changes than before. In the treatment of IE, the principle is antiinfection and removing vegetation. Indications of surgical treatment: severe heart failure, unmanageable infection, large valve vegetations. The rate of IE complication is high and the overall prognosis is acceptable.
