【Abstract】In recent years, more and more rheumatologists and medical personnel have invested in the chronic disease management of rheumatic disease. As a pioneer in this field in China, our department has been exploring and practicing chronic disease management for more than 10 years. It has provided thousands of patients with "extended" medical services and "building block" patient education. Patient compliance and prognosis were significantly improved to achieve better survival. We believe that adhering to the chronic disease management of rheumatic disease can further improve the patient's treatment effect, medical experience and satisfaction, reduce the overall medical cost of society, and gradually align and satisfy the "value" of patients, medical staff and the government. It has great social significance.
