    【摘要】目的 探讨椎体成形术(PVP)与椎体后凸成形术(PKP)治疗老年骨质疏松椎体压缩性骨折的临床效果。方法 纳入杨凌示范区医院疼痛科2008年11月~2017年2月收治的老年骨质疏松椎体压缩性骨折住院患者284例,按照治疗时间先后顺序随机分为PVP组(135例)和PKP组(149例)。统计分析两组患者术后疼痛改善、骨水泥渗漏、椎体后凸角度改善及疗效。结果 两组患者术后VAS 评分、椎体后凸角度改善均优于术前 (P<005);术后PKP组VAS 评分、椎体后凸角度改善、临床疗效均优于PVP 组(P<005);PKP组骨水泥渗漏率明显低于PVP 组(P<005)。结论 骨质疏松椎体压缩性骨折应用PKP与PVP均可缓解疼痛,改善椎体后凸角度,但PKP临床疗效更好,骨水泥渗漏率更低,可为临床治疗提供借鉴。

    【Abstract】 Objective Objective To compare and analyze the clinical effect of vertebroplasty (PVP) and percutaneous kyphoplasty (PKP) in the treatment of osteoporotic vertebral compression fractures in elderly patients, so as to provide reference for clinical treatment. Methods 284 patients with osteoporositic vertebral compression fracture were hospitalized, and randomly divided into the PVP group and the PKP group. The postoperative pain improvement, bone cement leakage and posterior convex Angle of vertebral body were statistically analyzed. Results The VAS score and kyphosis angle of the two groups were all better than those before operation (P< 005). After operation, the VAS score and kyphosis angle of PKP group were better than those of PVP group (P< 005). The leakage rate of bone cement in PKP group was significantly lower than that in PVP group (P< 005). Conclusion PKP and PVP can relieve pain and improve kyphosis angle in osteoporotic vertebral compression fractures, but PKP has better clinical effect and lower bone cement leakage rate.
