低值血小板报警信息在不同检测系统的结果分析

    【摘要】 目的 根据血细胞分析仪血小板报警信息探讨Sysmex XE2100的电阻抗法(PLTI)、光学法(PLTO)、Sysmex XN3000的核酸染色法(PLTF)及Cella Vision DM96在低值血小板计数中的应用。方法 随机选取182例用 XE2100全自动血细胞分析仪电阻抗法测出血小板计数<100×109/L的标本,将所选取的标本2 h内采用 XE2100全自动血细胞分析仪的光学法、XN3000血细胞分析仪的核酸染色法进行血小板计数,然后用Sysmex sp1000i自动推片染色仪对标本推片并进行瑞氏染色,再用DM96自动血细胞形态分析仪计数血小板,最后按报警信息血小板直方图异常、血小板减少、血小板聚集将标本分为三组,分别对PLTI、PLTO、PLTF及DM96四种方法计数得到的血小板进行统计学分析。结果 在血小板直方图异常、血小板减少、血小板聚集三组报警信息中,仪器提示血小板直方图异常和血小板聚集报警信息时,PLTO和PLTI差异无统计学意义 (P>005),PLTO和PLTF,PLTI和PLTF, PLTO和DM96,PLTF和DM96, PLTI和DM96差异有统计学意义 (P<005),在血小板减少报警信息时,PLTO和PLTI,PLTO和PLTF,PLTI和PLTF差异无统计学意义 (P>005),PLTO和DM96,PLTF和DM96, PLTI和DM96差异有统计学意义 (P<005)。结论 在低值血小板(<100×109/L)计数时,根据不同报警信息的提示应选择不同的计数方法,在仪器提示血小板直方图异常和血小板聚集报警信息时,网织红通道的光学血小板计数法不能纠正血小板计数,必须用核酸染色法(PLTF)或DM96血小板计数,而仪器提示血小板减少报警信息时,必须通过DM96血小板计数复检才能发出报告。

    Abstract:

    【Abstract】 To explore the application of platelet alarm information of Sysmex XE2100 electric resistance method (PLTI), optics (PLT), Sysmex XN3000 nucleic acid staining method (PLTF) and cella vision DM96 at low values of platelet count. Methods182 patients with platelet count <100 * 109/L measure by XE2100 automatic blood cell analyzer (impedance method) (PLTI) were selected. The specimens from the patients were detected with XE2100 automatic blood cell analyzer (optical method) (PLTO), XN3000 blood cell analyzer (staining method) (PLTF), DM96 automatic blood cell morphology analyzer and Sysmex sp1000i automatic push piece dyeing instrument (DM96). The alarm information was divided into abnormal platelet histogram (Plt Abn Dst), thrombocytopenia (Thrombocytopenia) and platelet aggregation (Plt Clumps). The correlation of PLTI, PLTO, PLTF and DM96 was analyzed. Results The abnormal imformation of platelet histogram and platelet aggregation of PLTO and PLTI was not different (P>005). There were differences abnormal imformation of platelet histogram and platelet aggregation between PLTO and PLTF, between PLTI and PLTF, between PLTOand DM96 and between PLTI and DM96 (P<005). There were no diferences of thrombocytopenia between PLTO and PLTI, between PLTO and PLTF and between PLTI and PLTF (P>005). There were diferences of thrombocytopenia between PLTO and DM96,between PLTF and DM96 and between PLTIand DM96 (P<005). Conclusion In the low value of platelet (<100* 109/L) count, the different warning information should choose different counting methods. In the instrument tip PLT Abn DST and PLT Clumps alarm information, the reticulocyte red channel optical method of platelet count cannot be corrected platelet count must be by nucleic acid staining method (PLTF) or DM96 platelet count, and the instrument tip thrombocytopenia alarm information must use DM96 in order to pass the inspection of the platelet count issued a report.
