Abstract:【Abstract】 ObjectiveTo investigate the effect of BiPAP on blood gas and work of breathing in patients with AECOPD respiratory failure. Methods 80 patients with chronic obstructive pulmonary disease acute attack combined type II respiratory failure were randomly divided into treatment group and control group which contained 40 cases in each group according to the simple digital table. Both groups were given conventional treatment, besides treatment group using BiPAP ventilator additionally for mechanical ventilation. Blood gas analysis and the changes of respiratory dynamics before and after treatment were observed. Results The respiratory rate, dyspnea and heart rate of treatment group was better than that of the control group (P<0.05). There was no effect on blood pressure in treatment group and control group (P>0.05). After treatment, the arterial oxygen branch pressure (PaO2) of treatment group and control group were significantly increased, compared with those before treatment (P<0.05), while the treatment group increased more significantly. Compared with that before treatment, the treatment group arterial carbon dioxide partial pressure (PaCO2) after treatment were significantly decreased (P<0.05). The breathing work of patients after the treatment was decreased significantly, compared with that before treatment (P<0.05). There were no serious complications. Conclusion The exact BiPAP ventilator in treatment of ACOPD merger of respiratory failure patients can effectively improve the clinical symptoms, arterial blood gas analysis, increase respiratory muscle function, do not significantly affect the hemodynamics and help restore the health of the patients.
