【Abstract】 Cardiac rehabilitation is increasingly important. After cardiovascular disease events, cardiac rehabilitation and medication are two complementary aspects. As an integral part of the treatment and care of cardiovascular disease, cardiac rehabilitation is gradually updated and standard. The measures of Cardiac rehabilitation transitional care mainly includes comprehensive nursing assessment, sports training, smoking cessation, the management of weight, blood glucose, blood pressure and lipid , lifestyle changes, nutritional counseling, psychological nursing and health education, and so on. In this paper，the latest advances for cardiac rehabilitation transitional care in coronary heart disease patients with PCI at home and abroad was reviewed.
